All applications to the charity for financial assistance must be received on an official application form.  

You should use the Relief-in-Need application form if you are an individual and require financial assistance with any items of extraordinary expenditure that you cannot afford.

If you are representing an organisation which requires financial assistance - whatever the amount – you should use the Grant application form.
